Health Insurance

BHG will continue to offer medical coverage through Anthem. For the 2026 plan year (January 1, 2026 – December 31, 2026), there will be a minimal 5% increase in premiums.

  • Option 1: ACA-qualified deductible plan — $6,350 individual deductible
  • Option 2: PPO 1000 (co-pay plan)
  • Option 3: PPO 500 (co-pay plan)

Supplemental Benefits

Supplemental voluntary benefits (100% employee-paid): Life Insurance, Critical Illness, Accident.

New: Short Term Disability, Flexible Spending Account (FSA), and 2026 Childcare limit increased to $7,500.

Vision & Dental Coverage

Vision: Voluntary Anthem plan offering $10 copays for exams and materials, plus frame/contact allowances.

New Dental: Coverage transitioned from Principal to Delta Dental.

Hourly team members pay 100% of premiums for these plans.

Eligibility & Payments

Employees become eligible after 1 year of continuous employment and an average of 30+ hours per week. Enroll by the end of the month following your anniversary or during open enrollment.

Important: Failure to pay premiums within 30 days results in removal from the plan. Eligibility is rechecked each October.

401(k) and Roth IRA

Employees may be eligible if they meet the following criteria:

  1. Are 21 years of age or older.
  2. Have completed 1 year of service.
  3. Have worked at least 1,000 hours.

Company match:

  1. 100% match on the first 3%.
  2. 50% match on the next 2%.
  3. Up to a 4% company match.

All employees interested in participating must submit their interest by June 21, 2026 for a potential July 1, 2026 effective date.
